Wraparound Facilitation
Wraparound is a comprehensive case management service that “wraps” the entire family in care. Working with a wraparound facilitator, the family identifies their strengths, and needs, then establishes goals, and begins to learn about resources with their community. They develop an individualized wraparound team from extended family, formal and informal social supports, and members in their community. They work with their team to break their goals down into specific steps, tackle obstacles that arise, and update goals as old goals are met. The wraparound facilitator is instrumental in helping to form the team and identifying the family’s goals. However, the facilitator only focuses on teaching skills; once the team is fully functional, the facilitator will leave and the team graduates to work independently going forward.
